This study aimed to determine the nationwide prevalence and predictors of gestational diabetes mellitus (GDM) in Turkey. A prospective screening was conducted among 2643 pregnant women from 51 centers across 12 regions between August 2016 and November 2017, using a two-step method and Carpenter and Coustan criteria. The national GDM prevalence was 16.2% (95% CI 15.0% to 17.4%), with no significant urban-rural difference. Women with GDM were older (32 ± 5 vs. 28 ± 5 years) and had higher BMIs (27.2 ± 5.1 vs. 24.7 ± 4.7 kg/m²) than those without GDM. Maternal age, BMI, history of previous GDM, and family history of diabetes were significant predictors. Low-risk women (age < 25, BMI < 25, no family history) comprised 10.7% of the population, with a GDM prevalence of 4.5%. These findings highlight the need for implementing international guidelines for GDM screening and management in Turkey.
